State Lawmakers Support State Medicaid Takeover Proposal

TITLE OF BILL:
An act to amend chapter 58 of the laws of 2005, relating to authorizing reimbursements for expenditures made by or on behalf of social services districts for medical assistance for needy persons and the administration thereof, in relation to calculating social services district
medical assistance expenditure amounts
PURPOSE:
This bill provides for an immediate freeze of local contributions to the cost of Medicaid followed by the gradual state assumption of all non- federal Medicaid costs over an 8-year period.
SUMMARY OF PROVISIONS:
Sections 1 and 2 of Part C of Chapter 58 of the Laws of 2005 are amended to provide a phased takeover of the local share of Medicaid costs.
JUSTIFICATION:
Since the enactment of the Medicaid program in 1965, local social service districts (counties and the City of New York) have been required to share in both the cost and administrative operation of the program. Over the years, the cost of the program has grown exponentially while responsibility for program design and administration has been gradually shifted to the state. By 2016, all administrative responsibility for Medicaid will be assumed by the state.
Despite this shift of responsibility, county and New York City governments continue to be required to contribute a significant share of the total program cost. Currently, counties and New York City pay approximately $7.3 billion per year and under existing law, these costs will continue to increase by 3% each year. With the recently enacted 2% cap on property taxes, counties simply cannot continue to meet this state mandate without slashing other essential local services such as senior services, veteran's services, law enforcement, parks, and road maintenance.
This legislation provides for the gradual assumption of the current local shares starting with an immediate freeze on local contributions effective January 1, 2012. On October 1, 2012, local contributions will be reduced by 5%. Additional reductions will be made in the subsequent years ending the with the full assumption of local Medicaid costs in 2019.
The timing of this legislation is designed to take advantage of changes that may be required by the federal Affordable Care Act as well as state actions being implemented or planned by the Governor's Medicaid Redesign Team. Given the phased nature of the takeover, there is ample time for the Governor and Legislature to develop and consider changes to the program that may be required to maintain program compliance with federal law as well as affordability to the state.

WASHINGTON – Cost estimates for a key part of President Obama's health care overhaul law have ballooned by $111 billion from last year's budget, and a senior Republican lawmaker on Friday demanded an explanation.
House Ways and Means Committee Chairman Dave Camp, R-Mich., wants to know by Monday why the estimated ten-year cost of helping millions of middle-class Americans buy health insurance has jumped by about 30 percent.
Administration officials say the explanation lies in budget technicalities and that there are no significant changes in the program.
The revised numbers, buried deep in the president's budget, stumped lawmakers and some administration officials for most of the week. At a congressional hearing Tuesday, Health and Human Services Secretary Kathleen Sebelius, who is in charge of carrying out the health care law, indicated she was unaware of the changes.
At issue are subsidies that will be provided under the health care law to help middle class people buy private coverage in new state insurance markets that will open for business in 2014.
Last year's budget estimated the cost of the aid to be $367 billion from 2014-2011. This year's budget puts it at $478 billion over the same time period.
"This staggering increase ... cannot be explained by legislative changes or new economic assumptions, and therefore must reflect substantial changes in underlying assumptions regarding the program's ... costs," Camp wrote Friday in a letter to Sebelius and Treasury Secretary Tim Geithner.
Republicans say they're concerned that either the estimated cost of the insurance has gone up, or that the administration has determined many more people will be losing employer coverage and going into the new government-subsidized markets, which will be called exchanges.
Administration officials say the big increase from last year's estimates is no cause for alarm and that the administration is not forecasting an erosion of employer coverage or higher insurance costs.
About two-thirds of the increase is due to effects of newly signed legislation that raises costs for one part of the health care law, but still saves the government money overall. The rest is due to technical changes in Treasury assumptions about such matters as the distribution of income in America.
"The estimates do not assume changes in what exchanges look like, the cost of insurance, or the number of Americans who will get their insurance in this new marketplace," Treasury spokeswoman Sabrina Siddiqui said in a statement Friday.

President Obama's national health care law will cost $1.76 trillion over a decade, according to a new projection released today by the Congressional Budget Office, rather than the $940 billion forecast when it was signed into law.
Democrats employed many accounting tricks when they were pushing through the national health care legislation, the most egregious of which was to delay full implementation of the law until 2014, so it would appear cheaper under the CBO's standard ten-year budget window and, at least on paper, meet Obama's pledge that the legislation would cost "around $900 billion over 10 years." When the final CBO score came out before passage, critics noted that the true 10 year cost would be far higher than advertised once projections accounted for full implementation.
Today, the CBO released new projections from 2013 extending through 2022, and the results are as critics expected: the ten-year cost of the law's core provisions to expand health insurance coverage has now ballooned to $1.76 trillion. That's because we now have estimates for Obamacare's first nine years of full implementation, rather than the mere six when it was signed into law. Only next year will we get a true ten-year cost estimate, if the law isn't overturned by the Supreme Court or repealed by then. Given that in 2022, the last year available, the gross cost of the coverage expansions are $265 billion, we're likely looking at about $2 trillion over the first decade, or more than double what Obama advertised.

Why do you say that??? ~ People with pre existing conditions who couldn't get any coverage... now can BUY their own insurance. ~ Kids who can't find a job with insurance after highschool or college, now can continue to be covered by their parents insurance PAID FOR BY THEIR PARENTS.
It amazes me that Conservatives are so concerned with the health of an uninsured zygote, up until the time that zygote becomes a human being... but once that human being is born... they abandon him/her and hand the baby the bill on the way out the door of the hospital.
