Student Weight Status Category (WSC) Reporting
2012-2013 WSC Reporting Information
When:
All Student Weight Status Category Surveys must be submitted via the Health Commerce System by Friday, January 25, 2013.
Who:
Districts Selected for 2012-2013 Reporting
Lists school districts who are required to report aggregated information from school health certificates/appraisals for students who were in pre-Kindergarten or Kindergarten and in grades 2, 4, 7 and 10 in the previous school year (2011-2012).
BOCES, Charter, Private and 4201 designated schools are not required to report WSC data. New York City schools are exempt from state education law that requires this data to be reported to the State Health Department. The New York City Department of Education has developed a system of monitoring students to determine weight status using NYC FITNESSGRAM.

You will have the oppportunity to ask questions while viewing the forms online. All calls are from 3:00 - 4:00 PM

Section 5: Notification Resources
Parental Notification
Schools may provide notice to parents via the school newsletter, website, the pre-Kindergarten or Kindergarten registration packet, and/or other reasonable means. The forms below are for parent use who wish to opt out of reporting. It is suggested you inform parents that any opt out forms should be returned by January 2nd, 2013 as the survey will be due for submission by January 25th, 2013.

Health Provider Notification
It is reccommended to follow up with the health care provider if they omit the WSC from the Health Appraisal, but not required that you do so.
